Sacramental Records

Our Lady of Perpetual Help records all sacramental celebrations which take place in the parish. Our records span the life of the parish.
 
The parish is able to provide proof of sacraments celebrated here as well as provide letters to other churches for OLPH parishioners who wish to serve as godparents or sponsors.

Often, godparents or sponsors for confirmation are asked to provide a letter stating suitability to act in this role. Registered parishioners should download the appropriate form, read, sign, date and return it via email to the parish secretary at [email protected] . Include in your email the requesting parish’s contact information and mailing address.

If you need a current copy of your baptism or marriage certificate, email your request to the parish secretary at [email protected] with an address and a contact name. Certificates will be scanned and emailed to the requestor unless otherwise specified.  Provide dates of birth, birth name, mother and father names, and if the baptism was much later than usual an approximate date of baptism,  in order to identify your record correctly. For marriage certificates please provide the full names of the spouses and the date of marriage.

Please allow at least two weeks for your request to be processed, and note that requests made electronically are dealt with more expeditiously than those made by phone or mail.
